[English]

The modern medical institution is an arena of social action and the result of the interaction between the doctor and the patient, and in this arena, the behavior of the actors is faced with a percentage of error and harm. This trend is a significant challenge in global health systems that has widespread consequences. The aim of this research is to understand how patients encounter medical error. Therefore, a qualitative approach and a data-based method were used. The data are the result of semi-structured interviews with patients and families who experienced medical errors. Participants were selected through purposive and theoretical sampling. The results show that the causal conditions fall under three main categories; limited medical human capital, institutional inefficiency, and disorganization of the professional role. The three factors of citizen neglect, active-passive participation, and irrational and anomic selection are the underlying and intervening conditions; self-help in the face of crisis, identity reconstruction and restoration, and self-selected relief are the three strategies for managing the situation that people involved in medical errors have used. According to the findings, it is suggested that structural changes, the establishment of independent monitoring systems, improving medical education standards, and strengthening transparency and accountability in the healthcare system be undertaken to correct this situation, as well as supportive interventions for affected patients, public education in the field of informed physician selection. With this process, we can stop the trend of moral inversion and move the health system towards justice, efficiency, and accountability.
